Physical therapy is billed per 15-minute unit, not per session, and one appointment usually generates several units across several codes. The evaluation that starts a course of treatment is priced separately and by complexity.

Nearly every “average cost” figure you will find for this procedure traces back to sites citing each other rather than to data. This one does not publish a number it cannot source. What it does instead is better: below is how to get the actual price your provider has already published — federal rules require it to exist, and it is specific to the facility that will bill you.

How to find the real price for you

    Get the code from your order

    Most PT codes are timed in 15-minute units — therapeutic exercise is 97110, manual therapy 97140, neuromuscular re-education 97112. The initial evaluation is 97161-97163 by complexity level. A one-hour session can be four billed units across two or three codes.

    Open the hospital's standard charges file

    Every hospital must publish one, usually linked from a page called “price transparency” or “standard charges.” Search it for the code — not the words — and look for the column marked discounted cash price. Ignore the gross charge: that is the chargemaster figure almost nobody pays.

    Watch for this specifically

    Ask the clinic for its cash rate per visit rather than per unit, because many offer a flat self-pay price that sidesteps the unit arithmetic entirely — and it is frequently well below what unit-billing would produce. If the clinic is hospital-affiliated, search the transparency file for 97110 to see the per-unit charge.

    Ask for it in writing, in advance

  • Units, not visits. This is the crucial mechanic: a session is billed in 15-minute increments, so the same appointment length can produce different totals depending on what was done.
  • How many separate codes are used in one session — therapeutic exercise, manual therapy and modalities are billed separately when all three occur.

If the bill has already arrived

A prepaid package of visits is a common and substantial cash discount at independent clinics. If a bill has already arrived, count the units against the length of your appointment — unit counts that exceed the time you were actually treated are a specific, checkable dispute.

Dispute a billing error

Before anything else, though: request the itemized bill and check it for errors. If the amount is simply unaffordable and the hospital is a nonprofit, the charity care screener is usually the more productive route.

Not a substitute, but often a large part of the plan. Ask the therapist to prioritise: which sessions genuinely need hands-on treatment or supervision, and which work you can do at home between them. A shorter supervised course with a solid home programme is a legitimate conversation to have.
